The 29-year-old model was the epitome of fall in a blue and green patchwork jacket over a white t-shirt. She paired that with khaki pants, brown shoes and a buttery yellow leather handbag. It's been a particularly busy week for the catwalk queen. She was spotted out on a girl's night out with her BFF Taylor Swift, 34, she celebrated her daughter Khai's fourth birthday and she walked in the Versace show at Milan Fashion Week.
